SHIPPING

  • Round-trip Ground shipping is INCLUDED in the rental price.
  • Equipment will be delivered to your door and picked up from your door.
  • Express/Air shipping is an additional cost to the rental price.

This equipment is FDA registered and UL rated, which means for your safety , Vitrectomy Solutions chooses durable medical equipment that has been tested under rigid governmental standards.

RENTAL PAYMENT

  • Rental charges must be prepaid before the equipment is shipped.
  • We accept major credit cards or debit cards: VISA, Master Card, American Express, Discover.
  • Rental time starts on Surgery date not ship date.

By accepting receipt of equipment, the Lessee agrees to the following rental terms:

  1. All equipment rental services rendered are charged to the Lessee and due at the time of service. The Lessee is responsible for all fees, regardless of insurance coverage. The credit card provided at the time of rental will be used to secure additional charges incurred due to late return or damaged equipment. You will be notified of such charges. NOTE: a $50.00 administrative fee will be assessed for any unsubstantiated credit card charge-backs.
  2. The Lessee agrees to maintain all equipment in good and sanitary condition for the Lessor, allowing for normal wear. The Lessor stipulates the support equipment has been examined and is, at the time of this lease, in good condition. If you do not return equipment in good condition, you will be charged the replacement price of the equipment.
  3. Upon receipt of support equipment lessee agrees to examine equipment. If any equipment appears to be missing or damaged Lessee must notify Lessor within 48 hours, or Lessee may be charged for damaged or missing parts of equipment.
  4. Lessee must notify Lessor immediately if surgery is cancelled, there is a change in surgical procedure, or if Lessee refused equipment delivery. Failure to notify Lessor of any of the aforementioned may result in additional charges. See equipment checklist for replacement pricing.
  5. For any unopened boxes that are returned to Lessor, Lessee will be charged for the cost of shipping and $20.00 re-stock fee per box if returned immediately. For opened boxes the first week’s rental is non-refundable. Federal Express Air shipping charges are non-refundable.
  6. At the expiration of the lease, the Lessee shall surrender all equipment in a good state of condition with all parts enclosed. Call Lessor office by 5:00 PM CST on or before the day of your Rental End Date to schedule your pick-up or rental time extension. If your rental ends on a Holiday, you must call by 5pm CST the business day prior to the Holiday. Lessor will make all pick up arrangements. Failure to call will result in additional fees and a daily rate charge incurred for every day following rental end date. Weekends will be added to rental charges if Lessee does not call on or before your rental end date.
  7. Save boxes and packing materials for return shipping. Lessor will schedule pick-up arrangements and will provide repacking instructions. All equipment must be made available for pickup on the next business day. If there is a problem with the Pick-up, please call Vitrectomy Solutions, DO NOT make arrangements with Fed-Ex or any other carrier.
  8. If the Lessee ships the equipment back to the Lessor without making return arrangements with the Lessor, or by the Lessee’s own means, the Lessee assumes all responsibility and any additional charges for the rental equipment.
  9. Boxes must be taped securely shut using packing tape or duct tape. If the box or boxes to the Lessor open in return shipping due to inadequate sealing or taping, the Lessee is responsible for any missing or damaged equipment or parts.
  10. A restocking fee of $20.00 per box will be charged to the Lessee if the equipment is returned with missing pieces, or pieces from other rental units. The Lessee is responsible to pack all parts in corresponding boxes upon return of equipment.
  11. If the equipment, or any part thereof is damaged or lost by any casualty other than normal wear, whether due to Lessee negligence or not, the Lessee will be charged for equipment repair or replacement. The Lessor will notify the Lessee of all additional charges. All additional charges added to the rental charge for the agreed upon rental period, will not exceed the purchase price of the equipment. Damage to the equipment includes, but is not limited to the following:
    • Make-up stains
    • Tears in Vinyl
    • Lost Cushions or Flannel Covers
    • Torn Carry Case
    • Cigarette Burns
    • Broken or Cracked Frame
    • Lost/Damaged Video
    • Smells Like Smoke
  12. If at any time the equipment is abandoned, the Lessee will be responsible for the cost of repair, replacement and transportation back to the Lessor.
  13. Vitrectomy Solutions, Inc. & Oakworks make no medical representation regarding the use of any equipment. Vitrectomy Solutions Inc. is not responsible for any medical problems that the Lessee may have which arise, or result in any way from any medical treatment of any kind including, without limitation, eye surgery rendered to the Lessee by a physician or other health care provider. Vitrectomy Solutions Inc. is not responsible for any injuries or damages which result from the use of the equipment in a manner in which the equipment was not intended to be used. The Lessee is encouraged to consult with his/her physician prior to use of the equipment.
