All Durable Medical Equipment rental services are charged to the patient and are due at the time of services, unless other arrangements have been made. You are responsible for all fees, regardless of insurance coverage. Pre-authorization and notification confirmation of benefits is subject to member eligibility on the date of rental, and any other contractual provision of the plan and may or may not reflect the actual amount of payment when the invoice is presented to your insurance provider.
